For customers using their own shipping containers — simply complete the form below, email your request or call us directly at (800) 431-1785. One of our customer service reps will schedule a UPS pickup. UPS will provide a pre-addressed airbill to affix to your package(s).

For customers using Kulzer-provided shipping containers — there is no need to complete this form. You may drop off your package(s) at a UPS Store/drop box or contact UPS at (800) 742-5877 to arrange a pickup — at no charge.
